Our mission is to be the Provider, Partner, and Employer of Choice.ResponsibilitiesResponsible for the performance and duties of clinical teammates being directed or assigned.Drive clinical outcomes.Set up monthly schedule.Perform direct patient care, including initiating and terminating dialysis, physical assessment of patients, taking and transcribing physician's orders, and drawing and documentation of ordered laboratory work.Participate in developing and implementing the patient's short- and long-term care plan and monthly progress notes.Involved in the interviewing and hiring process of new teammates.Qualifications1 year of licensed nursing experience6 months of dialysis experience (can be trained if you have other leadership or charge experience)3 days a week - 12hr shiftsMonday - Saturday (depending on clinic schedule)Some more details about this position:At least 6 months dialysis experience is required.or (can be trained if you have other leadership or charge experience)1 year of licensed nursing experienceMonday - Saturday (depending on clinic schedule)Coordinate ESRD patient care and services (trend lab values, track metrics on vital signs, weight, outcome management, etc.)* Ensure you and your clinical team are delivering the highest level of care to our patients by ensuring their safety, comfort, and well-being.Training may take place in a facility or a training clinic other than your assigned home clinicPotential to float to various clinics during and after your trainingYou must have a flexible schedule and be able to work mornings, evenings, weekends, and holidaysWhat you can expect:Build meaningful and long term relationships with patients and their family in an intimate outpatient setting.Be a part of a team that supports and relies on each other in a positive environment.
